That is usually due to an issue with the RX module on the Mortise case.
Please check the following:
The RX Module is not oriented correctly, should be on the inside of the door
The corresponding set screw is not installed on same inside of case as the RX module
The thin cable running along bottom of mortise case connecting RX Module is damaged
Micro switch in Module is broken / missing
Any of these would not allow the RX (request to exit) status which is required for both FDR and Commissioning.
