Why does my AD or CO series lock unlock when the lever is raised?

Installers have reported that their AD-CO Series cylindrical or mortise locks will unlock whenever the outside lever is lifted. 
 
This will occur if the outside spindle (square post) is installed without proper alignment of the "Triangles" of the spindle and housing.

This will occur if the cylinder and cam are miss-aligned during the installation of the cylinder and lever to the outside spindle.

This will occur if the cylinder tail-piece is miss-aligned during the process of re-pinning the cylinder for another key.
